Preparing for your visit
What to bring to an appointment -
- Insurance card and picture ID
- Insurance authorization to see specialist if your insurance plan requires it. (please check with your insurance if pre-authorization is required)
- List of current prescriptions and over the counter meds, along with dosage/frequency, as well as past prescriptions.
- Copies of MRI or CT scans, or XRays, or reports of blood work
- Copies of doctor or hospital reports
**New patient forms as well as any additional forms that a patient might be able to fill out ahead of time can be placed here as linked PDFs for the patient to print, fill out and bring to their appt.
** Insurance and Payment information –
any co-pays and deductibles will be the responsibility of the patient, due at the time of the office appointment.
